Goal Statement
The goal of this program is to improve the knowledge and competence of PCPs in the early screening, identification, and management of CKD 

Target Audience
The target audience for this educational program is primary care physicians, nurse practitioners and physician assistants/associates who care for patients with or who are at risk for CKD. 

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Construct a CKD screening and follow-up plan to facilitate early identification and management of kidney disease
  • Design a medication therapy plan for cardiorenal protection in a patient with CKD, integrating guideline recommendations with expanding evidence-based treatment options and patient specific clinical characteristics
  • Employ best practices in referral and multidisciplinary management of patients with CKD
