Goal Statement
The goal of this program is to improve the knowledge of learners in effort to optimize treatment for patients with Diabetes 

Target Audience
This program is intended for primary care physicians, nurse practitioners, and physician assistants/associates, pharmacists, and other HCPs who manage patients with T2D and overweight/obesity located in areas served by Area Health Education Centers (AHECs). 

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Session 1: Design an appropriate aim statement for a quality improvement project
  • Session 2: Develop a plan for improving performance on a quality measure for T2D and overweight/obesity
  • Session 3: Create a current state assessment using evidence-based tools such as process maps, fishbone diagrams, or driver diagrams
  • Session 4: Apply strategies to engage patients in nonjudgmental discussions on weight and individual weight loss goals
  • Session 5: Apply the key components of the PDSA methodology to making local changes
  • Session 6: Create a run chart to understand variation in data to determine and assess progress of improvement efforts
  • Session 7:  Apply the key components of the PDSA methodology to making local changes
  • Session 8: Design a treatment plan for a patient with T2D and overweight/obesity
  • Session 9: Apply the key components of the PDSA methodology to making local changes
  • Session 10: Outline the plan to sustain the intervention beyond the program period

Physician Associate Continuing Medical Education
