Goal Statement
The goal of this activity is to improve the knowledge and competence of learners in an effort to optimize treatment for patients with Heart Failure.

Target Audience
This activity is intended for cardiologists, endocrinologists, nephrologists, internal medicine physicians, primary care physicians, physician associates, nurse practitioners, pharmacists, and other members of the interdisciplinary team who manage patients with cardio-kidney-metabolic conditions. 

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Incorporate the latest clinical evidence into treatment decision-making for patients CKM) comorbidities, with a focus on individualized care strategies that enhance outcomes and quality of life

  • Foster meaningful patient engagement to promote safe, effective acceptance and adherence to CKM therapies

  • Implement coordinated, interprofessional team-based workflows that support the early and evidence-driven management of patients with CKM comorbidities
