Reshaping Diabetes Care: A Regionally Focused Quality Improvement Initiative to Optimize Care of Patients With Obesity and Type 2 Diabetes

This live webinar will highlight key insights and outcomes from a diabetes and obesity quality improvement (QI) program, featuring participants discussing their QI projects and lessons learned. Attendees will gain practical strategies to enhance guideline-based care, apply QI methodologies, and identify system-level opportunities to improve the management of type 2 diabetes and obesity in their practice.

Goal Statement
The goal of this program is to improve the knowledge of learners in effort to optimize treatment for patients with Diabetes.

Target Audience
This program is intended for primary care physicians, nurse practitioners, and physician assistants/associates, pharmacists, and other HCPs who manage patients with T2D and overweight/obesity located in areas served by Area Health Education Centers (AHECs).

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Summarize strategies to assess the impact of a QI initiative
